Established in 1978 as National Vision Services, it covers more than 2 million members in all 50 states and Puerto Rico. The company has regional offices in Arizona, Georgia, Iowa, Maryland, Massachusetts, Minnesota and Texas. Its provider network contains more than 18,000 carefully selected, screened and credentialed providers at nearly 10,000 locations nationwide. Approximately 70 percent of the network consists of independent providers, while the remaining 30 percent consists of national and regional chain providers. Avesis is headquartered in Phoenix.
