The Hartford Dispensary is a specialized program operated by the Greater Hartford Department of Mental Health and Addiction Services. The Hartford Dispensary-Intensive Case Management program provides a community based case management approach to individuals with co-occurring diagnosis of HIV/AIDS and mental illness. Its case management services include: home visits; clinical coordination of physician, psychiatric and health care services; individualized treatment plans with a full assessment of client strengths and needs; supportive client counseling; and client monitoring/reviews. The Hartford Dispensary is located at the Capitol Region Mental Health Center in Hartford, Conn.
