Founded in 1997, Mill Pond Health Campus is a part of Trilogy Health Services, which offers a range of community-based health care services. Mill Pond Health Campus is a full-service nursing care center that offers adult day care, assisted living, skilled nursing and memory treatment services. The center offers rehabilitation services, including speech, physical, occupational and respite therapies, home health management and hospice care. In addition, Mill Pond Health Campus provides services related to emergency and critical care, inpatient and outpatient surgery, and substance abuse and mental health management. The center also offers long- and short-term care, adult day health services, and assisted and independent living. It maintains a center in Greencastle, Ind.
