Rachel Rebecca Greenfield, MD

Jara Southerland, NP, received a bachelor of science degree in nursing from Ball State University. She received a master of science in nursing from Indiana Wesleyan University. She has been with Community Health Network since 2011, practicing in medical surgical units, primary care and urgent care settings. She has a special interest in primary care, internal medicine and acute care, as well as in preventive care, holistic care and chronic disease management. In her spare time, she enjoys walking her dogs, camping, swimming, and spending time with her husband and daughter.

Dr. Deig grew up in Evansville, Indiana. He attended Purdue University for his undergraduate degree in civil engineering. He went to Marian University-College of Osteopathic Medicine for Medical school in Indianapolis, Indiana. Following medical school, he completed residency at Union Hospital in Terre Haute, Indiana before moving to Memphis for a two-year family medicine/obstetrics fellowship. His interests in medicine include surgical and nonsurgical obstetrics, office procedures, and patient education. He loves the people he gets to meet through practicing medicine, the unlimited learning medicine offers, and delivering babies. He lives in Fishers with his wife Caitlyn, a labor room nurse. In his free time, Spencer enjoys riding bicycles, hiking, cooking and woodworking.

Dr. Arrizabalaga joined Community Group in 2012. She completed medical school at Harvard in Boston, Massachusetts and did her residency training in family medicine at Contra Costa Regional Medical Center in Martinez, California. Prior to moving to Indianapolis, Dr. Arrizabalaga practiced at Washington State University for six years. Her areas of interest include adolescent medicine and chronic conditions such as asthma, depression and anxiety. Dr. Arrizabalaga enjoys working with her patients to help them manage their health through problem-solving and working to find individualized strategies for better health.
