Along with our regular hours, we offer extended early and late hours several days a week. Trinity Health IHA Medical Group, Primary Care - Waterford is a designated Patient-Centered Medical Home (PCMH). PCMH patients and their families (when appropriate) establish a partnership with their practitioners to ensure they have the support needed to make decisions and participate in their own health care.
