Dr. Jose Simon Montelongo Jr. was born and raised in Houston, TX. He received his dual bachelor's degree from the University of Mary-Hardin Baylor and is a medical graduate from the University of North Texas Health Science Center, Texas College of Osteopathic Medicine. He completed his general surgery training at University Medical Center, Texas Tech University Health Science Center, El Paso, and is board-certified in general surgery. He completed two years of additional training at the prestigious Michael DeBakey Cardiovascular Institute at Houston Methodist Hospital, where he learned how to treat complex disease processes of the arterial, venous, and lymph systems. Dr. Montelongo's main interests are peripheral arterial disease, carotid artery disease, aortic aneurysms and dissections, venous disease, and dialysis access. His goal is to deliver high-quality and comprehensive vascular care to all his patients, as he takes pride in treating each patient as he would his own family member.

Dr. Faiza Malik is a board-certified family medicine physician who joined CLS Health on May 1, 2022. She earned her M.B.B.S. from Dow Medical College in Karachi, Pakistan, and completed her Family Medicine Residency at Mercy St. Vincent Medical Center in Toledo, Ohio.Dr. Malik specializes in preventive medicine, chronic disease management, and the treatment of common illnesses. She offers a wide range of services, including annual wellness visits, vaccinations, physical health screenings, and management of conditions such as diabetes, high blood pressure, high cholesterol, obesity, thyroid disease, asthma, and arthritis. Dr. Malik also treats minor illnesses like colds, the flu, strep throat, UTIs, and more.Located at the Clear Lake Campus in Webster, TX, Dr. Malik is committed to providing compassionate care with same-day appointments and on-site laboratory services for immediate testing. She also offers weight loss management and works with specialists to provide comprehensive care, ensuring a seamless healthcare experience for all her patients.
